Fetch paginated count for provider custom module forms

Arguments

active_status · Boolean

category · String

charting_context · ChartingContextEnum · Filters group note templates for the chart note being created. INDIVIDUAL excludes them. GROUP includes them while group charting is enabled and excludes them otherwise. When omitted, group note templates are not filtered.

include_default_templates · Boolean · Default templates are global form templates that are available to any organization (specifically Free Text and ADIME form templates)
